At Onyx Direct Care, we strive to provide the best care possible to all our patients. To ensure that we can accommodate the scheduling needs of our patients, we have implemented the following cancellation policy:

Cancellation policy

1. Appointment Cancellation

If you need to cancel or reschedule your appointment, we kindly request that you provide at least 24  hours notice. This allows us to offer your appointment slot to another patient in need of care.

2. Late Cancellation or No-Show

Failure to provide at least 24 hours notice of cancellation or not showing up for a scheduled appointment will result in a $25 fee. This fee is intended to cover administrative and operational costs incurred due to the missed appointment.

3. Fee Payment

The cancellation fee of $25 will be billed to your account. Payment for this fee is expected within 30 days of the missed appointment.

4. Repeated No-Show or Late Cancellation

Repeated instances of late cancellation or no-shows may result in additional actions, including dismissal from the practice or a requirement to prepay for future appointments.

5. Exceptions

We understand that emergencies and unforeseen circumstances can arise. If you have a legitimate reason for missing your appointment, please contact our office as soon as possible to discuss your situation. We will assess each case individually.

6. Reminder Notifications

As a courtesy, we provide appointment reminder notifications via phone, email, or text message. These reminders are sent to help you remember your scheduled appointments. However, the responsibility for keeping and managing appointments ultimately rests with the patient.

Please review this cancellation policy carefully. By scheduling an appointment with Onyx DirectCare, you acknowledge and agree to abide by the terms of this policy, including the $25 cancellation fee.
